Abstract. Surgut City in 2014 had a high prevalence of HIV — 1350.5 per 100 thousand population. Since 2007, due to changes in the regulatory framework for the prevention of vertical transmission of HIV we analyzed pregnancy outcomes in 193 women over the period 2001-2006 (retrospective research) and 499 women for the period 2007-2013 (prospective analysis). It is shown that carrying of pregnancy in women with HIV takes place in the context of high levels of specific infections, and is associated with the development of placental disorders (33.3% and 41.7% of pregnant women), premature birth (16.7% and 25.0% women, p <0,01) and the frequent transfer of newborns in the NICU — 33.3% and 25.0%, which is 2.7 and 7.7 times more children without HIV. The high degree of completeness of the implementation of the regional Protocol vertical HIV (antiretroviral prophylaxis / therapy and caesarean section without regard to viral load 38 weeks of pregnancy) influenced cha-simplicity of HIV transmission to the newborn and to minimize the risk of HIV infection of the child (0.0% 1.0%, respectively).
