Abstract. The direction of identifying the diagnostic significance systemic indicators immune inflammation in various diseases is intensively developing. The aim of the study: to evaluate the diagnostic potential of systemic immune inflammation indicators in osteoarthritis depending on the age of patients. Methods: A total of 179 healthy donors and 50 patients with osteoarthritis were examined at the outpatient clinic of the Regional Clinical Hospital No. 2 and the Veterans Hospital (Vladivostok). Blood parameters and the ratio of neutrophils to lymphocytes (NLP) and monocytes (NMP), platelets to lymphocytes (PLР), monocytes to lymphocytes (MLP), and the total number of neutrophils and platelets to lymphocytes (systemic immunoinflammation index, SIII) were analyzed. The correlation between the parameters and osteoarthritis was determined using multivariate regression analysis. Results. Healthy elderly individuals had an increase in the erythrocyte sedimentation rate and the content of large platelets, while, conversely, hemoglobin, its concentration in erythrocytes, hematocrit, and the number of erythrocytes decreased (p<0.05). A direct significant relationship was established between the number of neutrophils, monocytes, platelets and osteoarthritis (p≥0.01) and an inverse relationship with lymphocytes. A positive correlation was determined between systemic inflammation indices and osteoarthritis: NLP (HR: 1.02, 95% CI: 1.01–1.03), TLP (HR: 1.02, 95% CI: 1.01–1.03) and SIIV (HR: 1.03, 95% CI: 1.01–1.04), and a negative correlation with MLP (HR: 0.97, 95% CI: 0.96–0.99). Conclusion. The correlation between an increase in systemic inflammatory blood indices and osteoarthritis is additional evidence of the decisive role of immune inflammation in its progression. Determination of these indices, taking into account the age of patients, has promising clinical diagnostic potential.
