Abstract.

The article describes the case of nephrolitiasis, complicated with acute pyelonephritis in the patient, who received immunosuppressive therapy for ortotopic liver retransplantation for a long period. Inspite of calculus localization (the upper part of left ureter), signs of the left kidney obstruction and acute pyelonephritis the clinical presentation were not clear and typical, and demonstrated non specific inflammatory clinical and laboratory symptoms, that caused diagnostic difficulties. The etiology of the disease was established only after abdominal US and CT. Immediatlly started conservative treatment demanded more prolong period compare to non transplant patients. Thus, in patient with long period of immunosuppressive therapy nephrolitiasis, complicated with acute pyelonephritis may show no typical clinical presentations. At the same time, appropriate diagnosis and adequate therapy in this patient category determines recipient survival due to the high risk of sepsis.
